Hi. I recently tried to retrieve my vaccination Record through https://vaxcert.doh.ph,but I received a notification indicating that no records were found under my name.

The following are my vaccination details:
Full name: John Michael F. Fernandez
1st dose- 09/10/2021
2nd dose- 10/04/2021 lot no.20210611294
Mabalacat, Pampanga.
Additional Dose(Booster)- 03/11/2022 lot no.2100680
Magalang, Pampanga

I would greatly appreciate it if you could respond to my request.

Kindly coordinate directly with the National Patient Navigation and Referral Center (NPNRC) at Telephone number (02)8651-7800 local 2431-2345 or hotline number 1555 local 2 for immediate assistance.

Please prepare your reference number generated thru Vaxcert.doh.gov.ph for verification purposes and ease of transaction.
