While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

We regret to inform you that DOH does not have custody of all information that you are requesting. We ask that you coordinate with the Pampanga Provincial Health Office under the Pampanga LGU for this data requests
