Good day Ma'am/Sir,

I would like to follow up my record in COVID 19 Vaccination which it cannot be found in the tracker in your office. I am a teacher in Department of Education assigned at the Tabunoc Central School, Talisay City, Cebu, I was vaccinated last August 10, 2021 with vaccine manufacturer Janssen signed by Vaccinator Marie Shiela Selor with License No. 0779844. I am asking once again this record for the reason that I will be traveling to South Korea this coming November 11, 2023, I do hope you may steadfast my request for travel requirements.
