Good day! I would like to request for the following information:

1.The total number of positive cases of human leptospirosis for each year.
2.A table of all positive cases of human leptospirosis in Caloocan City that includes: 
  a) gender & age
  b) month & year they were diagnosed
  c) the diagnostic method/tool used, only if available. 
  d) treatment/medication (if any was given)

Ex: January 2013 (50 cases)
       Case 1: female, 27 years old, (diagnostic tool), (treatment)
