hello! i am in the process of researching about the procedure SHOCKWAVE LITHOTRIPSY for the years 2012 to 2017.

in view thereof, may i ask for the following data:

1.FACILITIES, providing SHOCK WAVE LITHOTRIPSY in the whole country for the years 2012, 2013, 2014, 2015, 2016 and 2017. please indicate 
A. name of facility, 
B. whether health facility is public or private.

may i also request to please kindly organize the data according to years indicated above.

may i ask that you also include facilities which are categorized as free standing clinics accredited by the DOH also offering shockwave lithotripsy for the years 2012, 2013, 2014, 2015, 2016 and 2017.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you requested. What we can provide to you as of the moment are the list of all the DOH accredited hospitals in the Philippines and the Administrative Order on their respective functional capacity, as explained in our previous transaction with you.
