I would like to request for my birth year to be corrected on the COVID-19 Vaccination Certificate issued in your system: VaxCertPH. Birth year indicated in the vax cert is 13 June 2003 instead of 13 June 2004.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. 

kindly coordinate with the LGU where you received your vaccination and request for assistance to correct the details in your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
