Hello! I am Courtney Kate Capit. I am fully vaccinated and I found my record on  https://vaxcert.doh.gov.ph/ 

Unfortunately, I had accidentally put a passport number while filling out the form in finding my vaccination record. But I don't have a passport number nor a passport. Is there any way that you can remove the passport number on my vaccination record?

You asked for Wrong Information on Vaccination Card.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
