Hi I would like to request another vaccine card. I have lost mine and do not have any picture. I do remember that I got vaccinated both times in fort Bonifacio Elementary school. The date I only remember is Nov 8 2021. I had difficulties because of my high blood pressure. I hope my request gets fulfilled because I have to use it in my work.

Your FOI request is approved. 
About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
