Hi,

I would like to get a copy of my vaccination certificate for all three doses. I had lost my vaccine card and I need it for international travel.

You asked for Vaccination Certificate Request for Lost Vaccine Card.

Response to Request

Your FOI request is approved. 

About the lost vaccination card, kindly coordinate with the LGU  where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card and vaccination certificate
