• Name and type of health facility
• Type of ownership
• Hospital bed capacity
• Number of personnel
• Name of typhoon
• Type of damage
• Number of injured personnel
• Cost of damage
• Cost of reconstruction or building
• Actual cost of repair, where applicable
• Repair status, where applicaple
