Hi! My name is Jona Jean Aguilar , born on Jan.11, 1992.
 I would like to request for the information or records on my childhood immunization. 

. Measles
• Mumps
• Rubella
• Polio
• Pertussis
• Hepatitis B
• Tetanus 

I was required to submit my records for my employment outside PH. Please do help me. My mother told me that I was vaccinated not in Baranggay since there is no vaccination on baranggays on that Time. We lost our record during Bagyong Sendong that hit Cagayan de Oro city last dec.2011. I am hoping that you could help me through this. Thank you and God bless!

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of immunization record. We ask that you coordinate with the Health Center  under the LGU where you received the immunization for this request
