Hi po.. Ako po si Diosdado Cardines Jr. 41yrs old.. gud morning po sa inyo lhat mga sir/mam..
Mginquire po sana ako sa inyo kung pde po ako mapasama sa mga PWD? Ako po ay naoperahan last april 2022 sa east ave med center.. ang sakit kpo ay PNEUMOTHORAX.. ngundergo po ako s VATS OPERATION.. collapse npo un right lungs ko

Kindly coordinate with the Municipal Social Welfare and Development Office (MSWDO) of the Rizal LGU and the MSWDO will encode your data at the Philippine Registry for Persons with Disability (PRPWD) system

Your membership for PWD will be based from the encoded data from the PRPWD system. Kindly coordinate with the MSWDO of the LGU for this request, thank you
