Hello!

I would like to request correction of my last name in my vaccination record and certificate on vaxcertph. 

The website / vaxcert record for my last name is set to  Ybanez as my lasr name but the correct  last name is Ibanez. I really need to change my last name asap because this is one of the requirements in applying for Angkas rider.
 
Below are all the details request to change.

Last name: Ibanez
First name: Aljeans
Middle name: Navos
B-day: December 18, 1992
Sex: Male

1st Dose: August 20, 2021
2nd Dose: September 12, 2021 
Booster: I forgot the date of my 3rd and booster shots

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
