Kindly Visit the National Health Facility Registry (NHFR) website at https://nhfr.doh.gov.ph/VActivefacilitiesList for the master list of health facilities in the Philippines. Select the health facility type per region and download the data in Excel or CSV format. Also, please accomplish the FOI Request Feedback form at the link https://bit.ly/DOHFOIFeedbackForm.

Your right to request a review
