I, LADY JOY GATPO CHING, would like to request to your good office regarding to my IMMUNIZATION RECORDS that I need for my US Vaccine Requirements for my US K1 VISA.

Thank you for your request dated Jan 02, 2024 10:26:08 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request

You asked for IMMUNIZATION RECORDS.

Response to your request

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of immunization record. We ask that you coordinate with the Health Center  under the LGU where you received the immunization for this request and specify the type of immunization you received and when you were vaccinated.
