I have lost my COVID-19 Vaccination Card. Would I be able to get another copy? I would be traveling next week. I hope we can work this out. Thank you.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
