Please be informed  that for Item Number 1, you can visit the DOH National Health Facility Registry (NHFR) website at https://nhfr.doh.gov.ph/VActivefacilitiesList for the master list of health facilities in the Philippines, select the health facility type per region and download the data in Excel or CSV format

For item 2 on  List and total number of all registered therapeutic and diagnostic medical imaging equipment, we shall refer this request to Food and Drug Administration ( FDA) for action.

1. List and total number of registered medical facilities (i.e., diagnostic center, hospital, etc.) in the Philippines.

- Visit the DOH National Health Facility Registry (NHFR) website at https://nhfr.doh.gov.ph/VActivefacilitiesList for the master list of health facilities in the Philippines, select the health facility type per region and download the data in Excel or CSV format

2.  List and total number of all registered therapeutic and diagnostic medical imaging equipment (such as LINAC, Brachytherapy, PET scan, MRI, CT scan, Mammogram, DEXA, Mobile and stationary x-ray, Dental X-ray) in the Philippines.
- The information requested may be assessed via the FDA Verification portal at https://verification.fda.gov.ph/Home.php and specific information (e.g. registration number, product name, classification) can be obtained. Please be informed that information reflected in the portal are the only information that can be shared.
