Good day! 

I would like to request urgently a medical assistance for the Hospital procedure of my MOTHER which we are lack of financial for the cost of angiogram, she currently diagnosed a CAROTICO CAVERNOUS FISTULA and we urgently need a CEREBRAL ANGIOGRAM that referred by her doctor neurosurgeon at PHILIPPINE GENERAL HOSPITAL. 

1. Urgently looking for free  CEREBRAL ANGIOGRAM procedure.
2. Need some medicine for the medication of my mother and need an assistance for hospital procedure.

You asked for LOOKING FOR FREE CEREBRAL ANGIOGRAM.

Response to Request

Your FOI request is approved. 

    1)  You are looking for  Free Cerebral Angiogram. We regret to inform you that we do not have data/records for the list of Hospital facilities that provides Free Cerebral Angiogram. However, we suggest to coordinate with the Philippine General Hospital and ask for referral if they have any. 

    2)  Please be informed that in order to request for medicine and funds for medical procedure, all applications for medical assistance are currently processed at hospitals under the Department of Health (DOH).
In order for your request to be met, we can go directly to the DOH or LGU hospitals that have Malasakit Centers. You can also contact the said facilities by phone or email. Just check the attached file and link below to find out the list of facilities with Malasakit Centers.
