Good day!

To whom it may concern,

I am writing to request for a vaccine certificate for international travel purposes. I have received a total of 4 doses of vaccines as follows:
03/10/2021: Astra Zeneca at Muntinlupa City
06/02/2021: Astra Zeneca at Muntinlupa City
12/02/2021: Pfizer at Muntinlupa City
06/06/2022: Pfizer at Calamba City

I tried to update my records last October 19, 2023 to add my 4th dose. Now when I try to get a certificate even for the 3 doses I cannot access that page. I need a certificate as a requirement for work and travel.
