Hi! I lost my vaccine card and I need it. I got my first dose in Polomolok, South Cotabato 8/28/21 and my second dose in Polomolok, South Cotabato 9/18/21. I got my third booster in Davao City, 2/16/22. I'm requesting for a replacement as soon as possible. Thank You!

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
