While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We regret to inform you that the Epidemiology Bureau (EB) - Field Health Services Information System (FHSIS) Unit does not collect data on the said requests. All data being collected by the FHSIS can be found at this link: https://doh.gov.ph/publications/serials
