Greetings! I would like to respectfully request for the FHSIS data from 1995 to 2020 in excel or csv format. The available data on the DOH website are in pdf version. I am currently drafting my thesis proposal for my Master in Statistics degree in UP-Diliman with a working title "Development of Local Health Service Delivery Index using Multivariate Analysis". The intent of the paper is to construct a comprehensive composite index on local health service delivery based on the most relevant indicators on service delivery, service delivery capacity, governance and financing.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We would like to inform you that the Epidemiology Bureau only releases data in PDF format. However, the 1995 to 2020 FHSIS Annual Reports (available on the DOH website) can be converted to Excel format using the software. You may search online for free PDF to Excel converters.
