Good day! I would like to request for some additional data found in the Annual Health Facility Statistical Report, Year 2019 Files of the following hospitals:

1.Isabela United Doctors Medical Center, Inc. located in Cauayan City, Isabela, Region II

2.De Vera Medical Center, Inc. located in Santiago City, Isabela, Region II

Namely:  
1.) Gross Death Rate

2.) Net Death Rate

3.) Infection Rate (Including rates for Ventilator Acquired Pneumonia, Blood Stream Infections, Urinary Tract Infection, Surgical site infections, if available)

4.) Staffing Pattern (Total Staff Complement)

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. We would like to inform you that the DOH Cagayan Valley CHD (DOH Regional Office 2) emailed/called & texted you the email addresses of said hospitals (Isabela United Doctors Medical Center, Inc & De Vera Medical Center, Inc.) together with their contact numbers for you to ask/inquire from them on the needed data for his research for the reason that some data are not available.
