Thank you for your request dated Nov 11, 2020 03:14:55 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Additional information from Annual Health Facility Statistical Report. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. We would like to inform you that the DOH Cagayan Valley CHD (DOH Regional Office 2) emailed/called & texted you the email addresses of said hospitals (Isabela United Doctors Medical Center, Inc & De Vera Medical Center, Inc.) together with their contact numbers for you to ask/inquire from them on the needed data for his research for the reason that some data are not available. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Undersecretary Lilibeth C.
