Hi! I would like to request for the updated vaxcertph certificate with "NO RECORD" of my 1st and 2nd dose. I already had my 1st dose dated November 27, 2021 and my 2nd dose dated January 03, 2022 in Balamban Cebu City but as i attempt to pull up my record it was still no record of my 1st and 2nd dose.

You asked for COVID-19 VAXCERT.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
