Good evening, I'm "Jefferson Fausto Jose", I'm asking for help to get a new vaccination card. 

I am fully vaccinated but lost my vaccination card. 

I hope it can be sent to me because this is one of the job requirements please.

Your FOI request is approved.
   - About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Your right to request a review
