Good day! I would like to request for the following:
1. Current status of hospitals and existing health institutions which offers pediatric care in hematology-oncology institutions in Naga City
2. The covering laws and policies on pediatric care institutions and the general guidelines in hospitals
3. Facilities required for hematology & oncology cases
4. Organizational chart of hospital and other healthcare institution
5. All related data to hematology and oncology cases
6. Morbidity and Mortality rate of Blood diseases and cancer cases in Naga City, Camarines Sur.

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested and do not collect data on the cases of Hematology and Oncology Diseases. Health statistics that DOH is collecting are found at the FHSIS annual reports and Philippine Health Statistics that are posted at the DOH website: https://www.doh.gov.ph/publications/serials
