Dear DOH Region 3,

I am writing to request the organization history of the health center in Barangay Balasing Santa Maria Bulacan. Specifically, I am interested in knowing the previous and current health personnel of the center, as well as the current number of personnel funded. Additionally, I would like to know how the national government funded the health center in Balasing Santa Maria Bulacan.

I would like to clarify that I will be using all information obtained for educational purposes. Your assistance in providing me with this information as soon as possible is greatly appreciated.
