Good day Ma'am/Sir,

I’m here again for another request . I would like to request for a cancellation of my vaxcert update request I created on May 22, 2023 (Booster 2nd Dose not included). I already sent my concern here and the FOI told me that they done encoding/update my request but  there still no progress in LGU regarding to my request. And I need to try to request again so that I can generate my vaxcert because I badly need the vaxcert for my travel.  Thank you for your kind consideration and fast action. Godbless.

You asked for Cancellation of Vaxcert Update Request.

Response to Request

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Don't worry, I already contacted Agent 182 from the Vaxcert Team and requested for assistance to generate your vaccination certificate. Please expect her to call you on your phone number or you can call them at (02) 8651-7800 local 2432.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph.
