Hi, I update my record in VaxCert PH and it seems it can't correct all of my request for my record. There's no record on my 2nd dose and until now I'm trying to use it for my correction. I request here because I need a faster processing due to having a immediate International flight. also I emailed vaxcertsupport and still no response. Thank you

My request are:
1. Need to correct my name, with the same as my Valid Id's
it must be John Andrewpaul C. Daliba

2. My 2nd dose is not reflecting on the System. I got my 2nd dose on October 25, 2021 Pfizer and on my Certificate it only shows 1 dose

Your FOI request is approved.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
