To whom it may concern, 
      My name is Enrico Tanciangco from 78 Camia St, Malaria Rd. Brgy. 185, Caloocan City. I lost my vaccine card  when I accidentally put it in the trash bag while cleaning my room last month .( September 2023) 
I would like to request a copy  of my vaccine card  as one of the requirement needed going to Hongkong this end of October 2023.  Thank you so much for your kindness and consideration.

vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
