Hi,

I need my vax certificate for my overseas travel on Sept 7. However, the record of my first vaccine was not recorded. I requested correction last Aug 14, and my request status remains open to this day. 

Correction Reference ID: 2017379

I have also sent an email to the vaxcertsupport@doh.gov.ph with an email subject: VaxCert Correction Request for Overseas Travel. My vaccination card is attached in the said email.

You asked for Vax Certificate.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
