Hi! I would like to request another copy of my booster vaccine card because mine got lost. I am fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Taytay, Rizal at Juan Sumulong Memorial Junior College. My 1st booster  was on July, 01 2022 under the name of  K. Arenasente I hope my request gets fulfilled for I will be using it for my employment. Thank you and Have a Nice Day!

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may contact the Vax Cert
Customer service. Here are the contact
details:

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
certificate.
