Last July pa ako nag email kay vaxcertsupport@doh.gov.ph pero wla pa. August 4,2023 nagmake request po aq sa efoi websites kya my tracking number po aq pero pag visit ko sa manila vaccine portal lagi pa din po yung Jackilou Pascua Addatu lumalabas..Nag request po aq for correction pra maging Jackilou Labang Pascua po kasi need ko po pang travel to work overseas po... Currently in Cagayan Valley po kasi ako matagal na akong wla sa Manila...Sana po ma settle na po pra ma download ko na po ung updated vaxcard

You asked for Vaccine Certificate Card.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
