May i request a copy or info of my vaccination card, because i have lost it.
Is there anything i can do also or requirements for the vaccine card?

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
