Hi. I would like to raise a concern re: my vaccination record. I lost my vaccination card last Feb 2023. I'm fully vaccinated and have booster shot but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. My first series of doses was at SMART ARANETA COLISEUM, conducted by QC LGU on June 16,2021 (1stdose- Sinovac) and July 14, 2021 (2nd dose - sinovac), my booster shot was also at SMART ARANETA COLISEUM conducted by QC LGU last Jan 19, 2022.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
