Hi! I would like to request an updated vaccine certificate. I have an upcoming travel overseas first week of September and I have already requested through vaxcert ph but still pending almost a year now but I didn't receive any reply or update. I also tried calling their hotline and email but it is not available. I hope you could help.

This is my information as well as vaccination dates.

1st Dose: MODERNA (August 28, 2021)
2nd Dose: MODERNA (September 25, 2021)
Booster: MODERNA ( January 28, 2022)

You asked for Vaccine Certificate.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
