Good day! 

I would like to request a validation on my vaccination. I requested my vax certificate online in your website but it shows that I have no record found in my 2nd dose.

I have my vax card with first dose November 30, 2021 and 2nd  dose on January 5, 2022 . But upon checking it on your website vaxcert.doh.gov.ph No record found in my 2nd dose.  Only 1st dose. I badly needed it on September 14 because it is needed for taking the board exam. My Certificate ID is: 536699821

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Rest-assured that we've already coordinated with Agent 228from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
