Good Day FOI,

I would like to request on DOH to update my vaccine record for Vaccination Certificate since my 1ST vaccination and booster shot does not yet appear. I also made the request for Vaccination Certificate last April 29, 2022 but until now, there is no update. I will be using this requested Vaccination Certificate to travel abroad by the 3rd week of November. 

Here are my details:
Complete Name: Ariel L. Cariño
Origin: Siayan, Zamboanga Del Norte
1st vaccination: Janssen 07/30/21 Disud, Sindangan, Zamboanga del Norte
Booster: Pfizer 06/14/95 Tag-Abaca, Basilisa, Dinagat Islands
