Response to your request

While our aim is to provide information whenever possible, in this instance We regret to inform you that we do not release medical records as this is sensitive personal information covered in the provisions of Data Privacy Act and under the Exemptions to the Right to Information under Memorandum Circular No. 89 issued by the Office of the President. 

For requests for death certificate, kindly coordinate with the Philippine Statistics Authority (PSA) on this request.

Kaso wala pong copy sa PSA.  pumunta na din po kami sa east avenue medical center kung saan namatay po ang nanay ko. Hindi na daw po sila nag bibigay ng record pag 15years na pataas nakakalipas. Gusto lang po sana namin malaman kung meron po kayong record ni OFELIA SIMBAJON GERILLA  na aking nanay. Malaki po kc problema namin ngayon dahil namatay na din po ang aking tatay at ang mga benefits nya ay ang aking nanay, hindi po namin makuha ang pension ng tatay ko po dahil wala kaming mapakitang dearh certificate ng aking ina. Para po sana sa mga kapatid ko tatay ung pension. Maraming salamat po kung makakasagot po kayo
