Ako po si SWEETHZEL SAMSON

  Pwede po bang mag request ng full copy ng vaccination card ko or kahit vaccination certificate po . Nakasama po kase sa sunog ng bahay namin yun vaccination card ko .. need ko po kase ng vaccert para sa oag aapply ng work.  
Hindi ko na po tanda yung date kung kelan po ako nakapag pa vaccine . Sana po mabasa nyp req. Ko ..salamat po ..

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
