Rerequest po ako ng new vaccine card or cert po dahil nawala po ang aking vaccine card matagal na po sa previous work ko po sa new na inapplyan ko at na hired po ako required po nila ang vaccine card sana matulungan nyo po ako.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
