Hi!
This is to request a booster certificate which was taken last 31 July 2023 at brgy 702, George Bocobo St. Malate Manila, for the purpose of traveling abroad.

My first dose and second dose was done in Dubai.

You asked for 1st Booster certificate.

Response to Request

Your FOI request is approved.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

 kindly coordinate with the Manila Health Department under the Manila LGU where you received your vaccination and request for assistance to generate your 1st Booster vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
