Good day Ma'am/Sir,

I would like to request for a cancellation of my vaxcert update request I created (1st  Dose not included). I just visited the Rural Health Unit where I got my 1st dose shot and they said that they already encoded it and I just need to cancel my update request . And also this is one of my  requirements to submit for my agency. With a reference ID no. 2046629. I already emailed vaxcertsupport@doh.gov.ph about this concern. Thank you so much.

Response to Request

Please be advised that we do not have custody of the VaxCertPH request transactions. You may call the National Patient Navigation and Referral Center for further assistance at (02) 8561 7800 local 2401 or 2431.

Your right to request a review.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph. Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you about the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
