I would like to raise a concern regarding my vaccination record. I lost my vaccination card last August 26, 2023 because my wallet was stolen and the said card was inside it. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in San Francisco High School, Quezon City. My first dose was on September 23, 2021 and the second dose was at October 21, 2021. As I have said above, I lost my vaccination card and wasn't able to take photo of it.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
