Hi I am Grant Cyeth Carl M. Rabina and I would like to ask where can I get a new vaccine card? I lost mine and I can't seem to complete the fill-up form on vaxcert since I forgot the dates I had my vaccine and I only remembered the places I had my vaccine. Can I still get a new one just in case and where can I get it from?

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
