I would like to request for a vaccination card because I lost it. Though, I still have a picture of my vaccination card. What will be the requirements for requesting a new vaccination card?

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.
