Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. However, you may wish to contact the Caloocan Health Department under the City Government of Caloocan at http://caloocancity.gov.ph/ for this information. We only have the list of DOH accredited health facilities posted at the DOH health Facilities and Services Regulatory Bureau website at http://hfsrb.doh.gov.ph/.
