Hi! I would like to request another vaccine card because mine got lost. I don't have any pictures of my vaccine card however I remember when and where I got vaccinated. I got my first dose December 3, 2021 I think in Freedom , Malaybalay City Bukidnon and my second dose at January 3, 2022 at the same place under the name Jacenth Ric Galleros.

Your FOI request is approved. 
About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
