1. Assessment tool for the accreditation of a facility for umbilical cord blood banking.
2. What are the other requirements aside from the assessment tool.
3. Laws in the Philippines that govern the collection of umbilical cord and establishing a facility for its storage.

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. The DOH accreditation is only for Blood Service Facility in which all issuances including the requirements are posted at the Health Facilities and Services Regulatory Bureau (HFSRB) website at https://hfsrb.doh.gov.ph/?page_id=18
