Your FOI request is approved. 

Attached is the  data on Morbidity Rate of the Top 10 Leading Diseases in the Davao Region

Please be informed that we do not collect data on terminally ill patients. We suggest you refer to the FHSIS Annual Reports posted in the DOH website at https://doh.gov.ph/publications/serials

For Hospice Care data, we also do not have custody of this data and suggest that you coordinate with the Department of Social Welfare and Development (DSWD) at http://dswd.gov.ph for this request
